Patient registration

DK-01

One record per person, one medical record number, consent recorded per purpose.

Identity

Everything here is on the record

Checked against this facility's records as you type.

Clinical basics and identifiers

Ayushman Bharat health account, if the patient has one.

Consent

One record per purpose

Each box below becomes its own consent record with its own purpose. Withdrawing one later does not withdraw the others.

The medical record number is allotted by CBS on save — the next in this facility's series. It is not typed here.

What the approved design draws that has no field yet

DK-01 as approved also carries a preferred language, an attendant or next-of-kin block, an insurance tab, and a claim code that links the patient's own Care app. None of those has a column behind it: RegisterPatientRequest accepts name, date of birth, sex, phone, email, address, blood group, external identifiers and consent, and nothing else.

They are listed here rather than drawn, because a field that silently discards what a clerk types is worse than a field that is honestly absent — and the insurance details in particular would be typed once at admission and never seen again.
